Job Description:

Congregational Home is seeking a Part-time LPN to work with our long-term care residents.* *Shift Time: *10p-6:30a* *Scheduled Shifts per Bi-Weekly Pay Period:* 5-6 shifts* *\*\*Shift Differential\*\* **Job Responsibilities:** Performs resident care duties, auditing units to ensure timely responses are given to resident needs.* Informs the resident in advance about care and treatment and any changes to the plan of care* Participates in nursing care assessments, planning, implementation and evaluation* Administers treatments in accordance with approved nursing techniques (generally, these units are staffed with a medication passer, for medication passing components)* Listens and contributes to shift-report, making special note to follow up on significant changes in condition, admissions, transfers, discharges, initiation or use of physical or chemical restraints, unnecessary use of any drug, incidents, unexplained injuries, medication errors, loss of resident property or evidence of resident or family dissatisfaction.* Partners with Nurse Care Manager to oversee Certified Nursing Assistants and Certified Medication Assistants in the implementation of plans of care to assist residents to attain or maintain highest practicable physical, mental and psycho social well being*Qualifications:** Must hold a current LPN license in the state of WI* Previous experience in a geriatric, rehabilitation facility preferred* Ability to work independently or with a team* Conducts oneself in a caring, sensitive and safe manner which promotes positive relations and rapport with residents and visitorsIf you are looking for a new opportunity and want to make a positive difference, consider Congregational Home and apply today!Job Type: Full-timePay: From $30.00 per hourBenefits:* 401(k)* 401(k) matching* Dental insurance* Flexible spending account* Health insurance* Life insurance* Paid time off* Referral program* Tuition reimbursement* Vision insuranceMedical Specialty:* GeriatricsPhysical Setting:* Long term careApplication Question(s):* Given the heightened health risks of the residents we serve, we are a fully vaccinated site, requiring COVID vaccination and annual flu vaccination of our employees, residents and visitors.
